Here are some sample questions you might be asked during the referral process:

• What medical conditions does the person or couple have?
• What are the care needs?
• What special equipment is needed (wheelchair, oxygen, commode, etc.)
• Is medication administration needed?
• Is there help needed with hygiene?
• Are there any memory issues?
• Are there food allergies or special diet restrictions?
• Are there mobility issues?
• Are there night-time care needs?
• Are there communications challenges such as speech or hearing?
• Are there location preferences?
• What financial resources are available?

There are some more specific and personal questions we need to ask in order to find the most appropriate facility. Privacy is essential for us and we only disclose information with permission to an approved facility.
